We are looking for enthusiastic individuals who have a zest for life, who want to make a huge impact in someone’s life and who isn’t afraid to get stuck in to make the individuals we support happy. Here at Cwmbran Day Opportunities, we help support adults with a range of learning and physical disabilities who want to get the most out of their day with meaningful inclusive activities.
One individual we support is Michael who loves spending time at Cwmbran Day Opportunities. His confidence has grown, and he loves to get involved in singing and meeting his friends here. Building relationships and friendships is key to what we do here. This is important for our work, family as well as those we support. We’re looking for some new support workers who can join our team. We’ll help you settle in and support you as you get to know us all. This isn’t really a job, it’s more of a lifestyle where you get to spend your days helping others to enjoy theirs. Key Responsibilities
- Individual Outcome and Well Being
- Within your defined service, ensure that you follow, deliver and participate in support planning which:
- Is detailed and person-centred, and enables everyone to become more independent and develop life skills. This must include choice in each area of support.
- Enables a proactive approach to the management of risks and that the individual is encouraged to take risks, which support personal growth in confidence and self esteem
- Encourages the individual to establish and maintain relationships, with friends and families who they chose to have a friendship and relationship with as little or as often as they would like. New friendships should be encouraged and facilitated
- Acknowledges and promotes diversity and is free from discrimination
- -s clear about the wishes and desires of the individual and has clear plans for what we are commissioned to deliver.
- Meets the legal requirements of the Health and Social Care Wales act, and any mainframe contract held with the commissioning body. It should also demonstrate best practice.
- The completion of person-centred daily records and enabling the team to find creative ways of communicating i.e. easy read information
- Where there are complex needs and behaviours that may challenge there is a clear plan which you will follow, based on functional evidence.
- Assist in the admission and transition of the people we, ensuring as far as possible that they are made to feel welcome, and to offer practical support during the settling in period.
That there is a robust activity plan in place and that you follow for everyone which:
- Identifies opportunities for development, training, education, employment, volunteering etc.
- That tries new experiences- and gives people autonomy and control over the things they want to do and try. That is flexible, and can be changed.
- That has a robust approach to eating and drinking and that encourages the individual to be more independent, develop cooking skills and menu planning or be involved with your support.
That the enable the views of the individual are routinely sought through regular engagement with the individual by
- Supporting at regular key-worker meetings
- That people are supported with their medication through:
- Being supported and encourage to self-administer medication where viable, using Salutem Policy and Procedures
- That the individuals are kept safe, and where possible free from harm. Where harm does occur that this is reported in line with the local safeguarding procedure, and in line with the regulator. This should be done with your team leader and service manager.
